A. Background

The Royal Australasian College of Surgeons (RACS), formed in 1927, is a non-profit organization training surgeons and maintaining surgical standards in Australia and New Zealand. RACS supports the ongoing development, maintenance of expertise and lifelong learning that accompany the surgical practice of more than 8,300 surgeons and 1,300 surgical trainees and Specialist International Medical Graduates. RACS’ vision is for safe surgical and anesthetic care to be available and accessible to everyone. Through partnerships with Timor Leste and Pacific neighbors, RACS Global Health provides specialist medical education, training, capacity development and medical aid to 15 countries in the Indo-Pacific region and aims to support local health care systems to develop so they can provide quality medical services.

The Department of Ophthalmology is based at, and forms part of, the Hospital Nacional Guido Valadares (HNGV) in Dili. It brings together eye health agencies operating in Timor Leste to provide comprehensive and high level surgical and outpatient eye service delivery by Timorese eye doctors and clinicians. The service includes vision screening; refraction and spectacles supply; cataract and other surgery in its twobed operating theatre; treatment trauma and eye disease; and eye health care education resources. In addition to being the major referral facility for eye treatment, it acts as the base for coordination of surgical outreach visits to district referral hospitals and community health centres (CHCs), and clinical attachment for eye care staff (also based in the districts). The HNGV Department of Ophthalmology is supported by the Royal Australasian College of Surgeons (RACS) and the Fred Hollows Foundation (FHF Australia).

B. Purpose and objective of the assignment

The HNGV Department of Ophthalmology is going through a transition period where the International Long-term Ophthalmologist/Advisor has left after supporting the center for ten (10) years. RACS Global Health and FHF Australia aim to help ensure the department continues its ability to function at a high level and effectively delivers eye care services (as the main referral centre in Timor-Leste) and to support district eye care centers throughout the country. This includes clinical and surgical outreach, and training activities.

RACS Timor Leste is recruiting a National Consultant to develop Primary Eye Care (PEC) Curriculum and Training Manuals (Trainer’s Manual, Participant’s Manual) in line with Instituto Nacional Saúde Pública Timor Leste (INSPTL) training package, to support Ophthalmologists and Postgraduate Diploma in

Ophthalmology (PGDO) doctors to effectively deliver PEC training activities for health professionals around the country. This consultancy will focus on the development of PEC training-related documents with an emphasis on developing the PEC Curriculum and PEC Training Manuals.

The National Consultant will work closely with RACS Timor Leste Interim Country Manager, Head of HNGV Department of Ophthalmology, and RACS Project Staff to undertake this consultancy. He/she will ensure that a comprehensive and context-relevant Primary Eye Care Training curriculum and training manuals are developed and that these align with the Timor Leste Ministry of Health (MoH) human resources priorities and guidelines and the INSPTL standard in-service training package to meet the Timor-Leste National Eye Health Strategic Plan (NEHSP) https://www.iapb.org/news/timor-leste-has-new-eye-healthstrategic-plan/.

C. Scope of the assignment

The National Consultant will work closely with the Head of HNGV Department of Ophthalmology and RACS Project Staff to:

  1. Review and analyze documents related to eye sector development and primary eye care service. Work closely with the NEC Clinic Manager, Ophthalmologists at HNGV Department of Ophthalmology to determine the most appropriate and relevant materials of primary eye care to incorporate into the curriculum and training manuals.
  2. Develop the outline/curriculum design based on the desk review results in consultation with the NEC Clinic Manager, Head of HNGV Department of Ophthalmology and RACS Timor-Leste Program Team.
  3. Work in collaboration with the NEC Clinic Manager, Head of the HNGV Department of Ophthalmology and all Ophthalmologists to develop and design the training manual for participants and manual for trainers.
  4. Work in collaboration with the NEC Clinic Manager, Ophthalmologists at the HNGV Department of Ophthalmology to support the revision of the PEC Curriculum and Training Manuals.
  5. Undertake consultation with Psychosocial Rehabilitation and Development East Timor (PRADET) and Asociação Deficiênsia Timor Leste (ADTL) in collaboration with HNGV Department of Ophthalmology and RACS Timor-Leste Program staff to review existing PEC Curriculum and Training Manuals
  6. Undertake consultation with INSPTL in collaboration with HNGV Department of Ophthalmology and RACS Timor-Leste Program staff to review existing PEC Curriculum and Training Manuals.
  7. Develop the PEC Curriculum and PEC Training Manuals for both participants and trainers working in collaboration with the NEC Clinic Manager, Head of HNGV Department of Ophthalmology and RACS Project Staff.

D. Description of the assignment

The Consultant’s key tasks are to:

  1. 1Perform desk reviews on PEC training related documents, INSPTL training manuals and training curriculum examples from the INSPTL.
  2. Map out the content design of the PEC curriculum and training manuals.
  3. Develop the PEC Training Curriculum and PEC Training Manuals.
  4. Deliver a presentation on the draft of the PEC Curriculum and Training Manuals to HNGV Department of Ophthalmology /Ophthalmologists and RACS team as well as to INSPTL.
  5. Deliver the final consultation report/ final PEC Curriculum and Training Manuals to RACS.

E. Objectives of the consultant

The consultant should approach the development of the PEC Curriculum and Training Manuals with the view to:

  • Working with HNGV Department of Ophthalmology and RACS Timor-Leste Program Staff to ensure the outputs of the consultancy are aligned with Timor-Leste’s health sector requirements.
  • Developing relevant and effective curriculum and training manuals that are in line with NEHSP 2021- 2050, in order to support the development of quality health professionals in the eye health sector.
  • Develop curriculum and training manuals for PEC to support effective training for health professionals.

F. Deliverables and timelines

The assignment will be for a maximum period of twenty (20) business days, and will deliver the following:

  1. Engage with RACS Timor Leste Team and RACS Melbourne to clearly understand the Scope of Services and results expected from this consultancy. An initial discussion with RACS collectively will be undertaken as part of the consultant selection process.
  2. Submit a proposal or outline of how the consultant will undertake the work (Scope of Services).
  3. Once engaged, the consultant will progress the work in close collaboration with the NEC Clinic Manager, HNGV Department of Ophthalmology and RACS Timor Leste, and with input from RACS Melbourne as needed.
  4. Submit a brief Inception Report based on the desk review. Outline the consultation approach, including a structured table of actions and dates that will result in the development of the PEC curriculum and training manuals.
  5. Submit revised brief Inception Report with feedback provided to RACS TL office for approval.
  6. Upon the acceptance of the Inception Report, with support from the NEC Clinic Manager, Head of HNGV Department of Ophthalmology, all Ophthalmologists and RACS Timor Leste Program Staff, deliver the consultation and develop the curriculum and training manuals.
  7. Submit the draft PEC Curriculum and Training Manuals for review by RACS.
  8. Present the draft PEC Curriculum and Training Manuals to INSPTL board members for review.
  9. Incorporate feedback from RACS on the draft versions.
  10. Deliver the final consultation report/PEC Curriculum and Training Manuals for acceptance by RACS

G. Qualification and Experience

Academic Qualification:

Relevant master’s degree or higher in health, education & training management, education, international relation and social studies field.

Experience:

  1. At least 6-10 years of progressive experience within the eye health/health, training and education development and/or international development environment.
  2. Experience in developing curriculum and training manuals.
  3. Experience in working in an international development sector. Previous work experience in similar assignments with I/NGOs and/or UN will be givenpreference.
  4. Preferred experience in clinic management especially in low-resource settings.
  5. Highly developed cross-cultural interpersonal and communication skills with a demonstrated
  6. capacity to build positive relationships and networks, working effectively across a broad range of stakeholders.

H. Reporting and Supervision

The Consultant will report to the RACS Timor Leste Interim Country Manager during the entire period of this consultancy, with oversight from the RACS Senior Program Advisor.

  • Sample work of one (1) similar health gap analysis or related health management piece of work.
  • Two (2) pieces of documented evidence, e.g., contract award or reference letter, from clients stating scope of services and deliverables related to previously completed similar assignments.
  • A brief methodology of the assignment/interpretation to be used during the assignment.
  • Work plan based on Table 1 required for the assignment.
  • Email and telephone contacts of at least two (2) relevant references.
